Get Scheduled
You can schedule an appointment either by calling our office at (763) 421-1905 or by booking online.
The Paperwork
After scheduling your appointment, please take a moment to fill out the appropriate online Health Intake Form, which can be found by clicking the link below. Alternatively, a paper copy may be filled out in-office if needed.
First Visit
On the first visit, we will go over your health history, perform an examination and any necessary x-ray images or scans, evaluate your goals, determine best care moving forward whether that be chiropractic adjustments/shock wave/lab testing/orthotics/etc. Our doctors will adjust on the first visit if deemed a good candidate.
